ABOUT
A framework is needed for health centers to organize their approach to patient-centered care. The patient-centered medical home model (PCMH) is a universal framework that provides guidelines for establishing care management, care coordination, access, and quality improvement. This assessment allows health centers to identify if they are meeting basic expectations to care for patients in a PCMH model. This fillable PDF tool identifies a health center's current status with processes such as team-based care, access, population health management, care management and care coordination, and performance improvement. Based on the results, health centers can develop plans to improve performance with the specific PCMH domains.
